Collaboration with caregivers is vital for person- and family-centered care, contributing to the overall stability of the health system. The Essential Care Partner Support Hub has been actively engaging with long-term care homes in Ontario since early 2023. This session provides long-term care and retirement delegates with practical insights and effective strategies to boost caregiver inclusion through resources, tools, and support from the Essential Care Partner Support Hub. Emphasizing opportunities for widespread adoption, the session aims to encourage the integration of caregiver-inclusive practices throughout the health system.
