The North York Congregate Access and Support Team (NYCAST), a partnership between North York General Hospital and eight LTC homes, enhances resident health outcomes through specialized services. This interactive workshop will showcase data from NYCAST, highlight barriers and facilitators to partnerships between LTC homes and hospitals, offer guidance on implementing similar partnerships in other communities, and discuss opportunities for future growth.
