Implementing Ontario’s Model of Care for Adults Receiving Palliative Care in Community Settings
Description
Many patients with life-limiting illnesses reside in long-term care homes. When residents receive palliative care, they experience better symptom management and quality of care, have fewer hospitalizations, and are more likely to have advance care planning discussions. Implementation of the model of care in community settings, including long-term care homes, aims to improve access to timely and appropriate palliative and end-of-life care. This session will focus on our implementation efforts to date to support broader integration and coordination of healthcare resources, at the provincial, regional and local levels, to deliver seamless palliative care to residents and their care partners.
